COASTAL MAMA AQUA REGISTRATION

You only need to submit this form once ever 😃

I take full responsibility for any occurrences in the pool. I take full responsibility for investigating and maintaining my own safety for this class including my ongoing suitability to participate and style of participation. At each class, I will inform/remind the instructor about any health conditions that are relevant. I have read, understood, acknowledge and agree to this declaration including the warning, exclusion of liability, release and indemnity. WARNING: Both exercise and pools carry risks of injuries. I have voluntarily read and understood this warning and accept the inherent risks. Participants may at times be photographed/filmed. These may be displayed on social media - typically 'stories' which disappear after 24hours. I am aware that I can opt out, but it is my responsibility to remind the instructor of my preferences. I understand that if I do not opt out, the instructor may use footage/images of me at their discretion.

Safety

If there is any significant medical condition or injury that could influence your safety in the pool:
  • e.g. epilepsy, heart conditions, acute/severe injuries...

  • discuss with your Dr / health professional to determine whether participation is suitable

  • pelvic floor issues or general aches and pains that are not the result of an acute injury should be manageable with modifications. But YOU are responsible for informing the instructor and modifying according to what feels safe to you during class, seeking any help required during class and organising individualised assessment on a separate occasion to ensure safety and optimal results.

  • you must let the instructor know at your first class, and keep them updated about any changes relevant to safety.

What about swimming ability?

Swimming ability is not essential for the class at Noosa Springs. However, if you don’t feel confident in deep water, you must let the instructor know, and remind her at each class. Most of the pool is easy to stand in and we always use floatation devices where it is deeper. Even in the deepest part, you could easily jump to the surface for a breath.

Hope to see you at the pool!
